Using a Google or Microsoft Form, create a form that outlines the feedback you seek. Share the link to your form. Need ideas - look at evidence posted, there are already some teachers in the district using it that will post evidence quickly! :)

I kept my form simple so teachers wanted to come back! I also opened it to people outside of our organization so visitors could participate
