I created a back to school getting to know you questionaire. I would use it to gather feedback and get to know my students better.

How I plan to use it

I plan to use this specific Google Form with the students on the first day to get to know them, as well as assess their ability to navigate through the computer.  Each morning the students will complete a Google Form as part of their Daily Check-In to assess their emotional well-being for the day, as well as anything they would like to share with me.  Their grammar, reading, and science quizzes will mostly all be given via Google Forms.

Plan for Forms

I plan on using Google Forms to gather information about my students at the beginning of the year.  It is also a great assessment tool.  I plan to use Forms for all of my vocabulary quizzes this year.

Using Google Forms

I have used Google Forms with colleagues for their birthday treat preferences, for voting purposes, and to gather information.
With students I use Google Forms for getting to know you questions and to find out what supplies they have available to them at home.

Functional Forms

I would use forms for a short pre-test in classroom to test prior knowledge on a topic.  I have used it with colleagues in the past to do surveys.  It is a useful tool.

Great tool

I like to use google forms as a formative assessment, warm up/review activity to see where everyone is at for understanding a particular topic.  I also could see using it as an exit ticket.

I also enjoy using forms to collect review book homework.  I simply put however many multiple choice questions as I'm assigning from a section of the review book, with generic 1-4 choices.  I don't bother to type out questions and answers since they are already in the book.  Students then put their answers in google forms and it is instantly graded for both me and the students.  Huge time saver.  It takes less time to set up the answer key then it does to go through and manually grade those types of assignments.

Forms

Google Forms can be used with my Creative Writing or STEAM Club students to collect data on preferred meeting times, activities we do, or how they're enjoying the activities so far.

Using Forms for Reflection

Google Forms is a great method for collecting student reflection data. I use Forms a lot to challenge the students to dig a little deeper into what they learned from a specific activity or unit of study. With the various question format options available, I can easily change up the way that students respond. Forms also allows me to analyze feedback data easily with charts.
