Create a Form

When you first get into your new Form, it will ask you (purple bar) to "Try the new Google Forms"...click on that. Change the title of your Form to "Forms Fanatic". Add Katie as a collaborator (under the 3 dots, upper right corner). Post the form's URL here as evidence. More Info

Customize

Click on the palette icon, and then the image icon in the bottom, right corner. Choose a header image that appeals to you. As evidence, explain how you would change the header to a photo that you have saved in your Drive. More Info

Questions

Insert a new multiple-choice question. Use at least two answer choices. Make this a required question. Add a YouTube video as your next item. Add one short answer about the video as your next item. As evidence, paste the link to the video you chose here. More Info

Responses

Take your "quiz" at least five times by clicking on the "preview" (eye) icon. When you are finished, view the results. Take a screenshot of your results and post it here as evidence. More Info
